Note: Fresh tissue (no fixative) must be delivered immediately to 8th floor lab CPA or Histology (OC.8.901) and given to someone's personal charge; identify it as a fresh specimen. Notify Histology before bringing any stat specimens to the Lab. For other specimens or frozen sections, see individual listings. Formalin fixed tissue may be delivered to the 8th floor lab CPA at any time.
For overnight processing, specimen must be in Histology lab and formalin fixed by 1700; slides will be prepared and reviewed the following morning. A completed surgical pathology request form including hospital number, name, date of birth, date of procedure, pre-op diagnosis, clinical history, ordering physician, and any other pertinent information must be submitted with the specimen.
After 1700, deliver specimens to the 8th floor lab CPA; notify CPA if it is a fresh specimen or a frozen section.
