Description
The Estimated GFR (eGFR) is not a test performed by the laboratory - it is a calculation that occurs via in EPIC. The result displays on the patient's chart as either Estimated GFR using the CKiD equation for patients <25 yrs, and the CKD-EPI (2021) equation for patients >25 yrs.
- For patients whose legal sex* is not male or female, a comment will be applied: “eGFR is a calculated estimate of glomerular filtration rate that uses patient’s age, height and legal sex in the calculation. eGFR is not able to calculate when the patient’s legal sex is designated as “male” or “female”.”
- *Legal sex is used as a proxy for “sex at birth.” Documenting legal sex is a required step at registration, and can only be changed by certain roles, which is the safest option in our current system.
- Race is not a variable in these calculations (updated on 12/19/2023).
The Estimated GFR value is created any time a serum Creatinine is performed by the laboratory AND there is an existing Dose Calc Height or Measured Height at the time the Creatinine test is performed. The rule will look back 60 days for a charted height on patients 6 months to 2 years of age, 90 days for patients 2 years to 12 years of age, and 365 days for patients 12 years and older. If the patient's height is charted after the Creatinine is performed, the eGFR will not be calculated.
NOTE: If an eGFR value is incorrect due to erroneous height value, the medical team must correct the height entry prior to contacting the lab to initiate a recalculation of eGFR
Lab: To correct the eGFR, "correct" the Creatinine result by adding a new result comment.
Note: On 12/19/2023, the eGFR (Expanded Schwartz) formula which requires both BUN and Creatinine was replaced by the CKiD formula, which only requires creatinine. The CKD-EPI equation for patients >25 yrs was also updated to the 2021 formula.
