Test Code LAB2929 CMV by PCR Quant, Newborn Blood Spot

Important Note

This testing is ONLY performed on the original Newborn Screening blood spot from the patient's time of birth. A new sample collection or add-on order is not appropriate for this test.

Instructions:

  • Ordering Provider/Care Team:
    1. ROI Form: Complete a Release of Information: Newborn Screening Specimen form (this form is for Washington DOH only). Please ensure that when completing the form with the family, that the birth mother's legal name at the time of birth is used in the upper section of the form (mother or other parent's current legal name should be used for signature in the lower section). If the patient is not a single birth, please indicate birth order as well (e.g., Twin #2). This form authorizes the State to release this sample to Seattle Children's Laboratory.
    2. Epic Order: Place a Future order for CMV by PCR Quant, Newborn Blood Spot (LAB2929) in Epic. Our Lab will activate the order once the sample is delivered to Seattle Children's Laboratory for testing.

The sample will be received by Lab as Community Service from the State Health Department. Newborn blood spot requests vary by State. Please see Newborn Blood Spot Requests: Information by State to determine the best contact for your request. The testing order will be available in Epic, which will be placed by provider listed on the Release of Information: Newborn Screening Specimen form accompanying the sample.
