Minimum Vol: 0.5 mL
Note: Every 24 hour dosing: Infuse IV over 30 minutes. Upon admission, initial levels should be drawn following the 2nd dose. Levels should be drawn at 2 and 6 hours after the end of the IV infusion. Document the date and the time of infusion and collections. Following a dose adjustment during a single admission, repeat 2 and 6 hour levels should follow the 1st dose of the new dose regimen.
