This test is for Seattle Children's employees only. Employees must complete the top section of the green Occupational Health request form before blood draw.

Container(s): Gold SST, Red or Gold Microtainer
Preferred Vol: 2 mL
Minimum Vol: 2 mL
Note: Enter employee ID number in Epic when prompted.
