Notify UW Microbiology Lab before sending the specimen (206) 598-6147. Send out Mon - Fri at 0900 and 1600 with the UW/HMC courier. Use a UW/HMC Microbiology test request form. Keep the last (green) copy for our lab records. UW forwards to Focus Labs.
