Container(s): Lt. Green/Mint Top Lithium Heparin, Lt. Green/Lithium Heparin Microtainer, Gold SST, Gold Microtainer, Dark Green/Sodium Heparin, Red

Preferred Vol:  1.0 mL

Minimum Vol:  0.5 mL

Note:  Draw trough level ½ hour before the start of an infusion. Note whether specimen is peak, trough, or random. As of January 2004, vancomycin peak monitoring is no longer recommended, per Pharmacy and Therapeutics Committee and Infectious Diseases. If drawing a peak, draw at one hour after the end of a 1 hour infusion.
